Better access to a doctor and the team of health professionals
Six GP practices are working together to make it easier for you to access healthcare professionals quickly. Trinity Care Wakefield gives you fast access to help, 7 days a week, every day of the year. On weekdays the service will be open until 20:00 which is good news for people who are working during the day and struggle to get a convenient appointment for an urgent concern.
Who is this telephone service for?
Call this number if you are concerned about your condition you feel you need to talk to a doctor or health professional today. Your condition does not need to be urgent or life-threatening.
If you feel you do not require a same day appointment, or you need an appointment for a routine matter, please call your own GP surgery directly.
Who will answer the phone?
A fully-trained nurse will answer calls to this service and they will have access to all of your medical notes. Trinity Care Wakefield is based within Trinity Medical Centre and all of the team work at local GP practices, where they know about the range of support and services available in Wakefield.
What will happen next?
You will be asked questions about your concerns and the nurse will find out more about the nature of your illness or problem. Once the nurse has talked to you and referred to your medical notes, you may then be given an appointment with a doctor for today’s surgery.
Not all calls to Trinity Care Wakefield will be given an appointment with a doctor. Sometimes you may be seen by a nurse at a minor illness clinic, or given advice about actions and treatments you can take at home, or recommended to visit to your local pharmacist.
The Community Support Team
Trinity Care Wakefield can also help in situations where someone is struggling at home and needs help quickly. If things suddenly change, through illness, an accident, or a carer not visiting, you can speak to the Trinity Care phone service who can send out a member of the Community Support Team. The Community Support Team are able to provide short-term support and help to put together the help needed, or advise on health services which could be of benefit to the patient.
